As we are not medical professionals we always advise our passengers to go to their local GP or travel nurse or travel clinic if you have one and then they will be able to tell you whether you need things such as malaria tablets and altitude sickness tablets.
I've had a look at the trip dossier on line and when you travel on the worlds highest railroad which gets to heights of 5,072m it states that the trains have been specially constructed to deal with the altitude, with sealed cars to maintain oxygen levels and heated water in the toilets to prevent them from freezing.
But its still advisable to seek advice from a medical professional.
