Visa Information for Indochina Adventure, 18/12/09

I need a few details for my visa applications, so can you please confirm the Cambodian point of entry for the tour? Also, the Cambodian visa entry form asks for "Person/Organisation to be visited". Should this be left blank, or is there a particular answer I should give there?

Similarly, for Thailand, I need to know the point of entry and any other relevant details such as the above.
